Welcome
Welcome to our practice and thank you for selecting our office to serve your dental needs. Our office is designed for your comfort and our staff is devoted to making your visit pleasant. We hope that your relationship with our office will be an effective and satisfying one for years to come.

Emergency Visits
We will make every effort to treat your immediate problem. Patients who enter this practice on an emergency basis should arrange for further treatment. The practice of making dental visits only when an emergency arises will not be acceptable in this office.

First Appointment
At the first appointment a thorough examination of all areas of your mouth will be done. This will include a detailed examination of the teeth, soft tissue, supporting structures, alignment, bite and more. All of this information will be carefully evaluated so that we may accurately access your dental needs.

Preventive Oral Hygiene Appointment
After your initial examination you will be instructed in a preventive oral hygiene session. During this time you will be made aware of the latest home care practices that correspond to your individual needs. Your teeth will be cleaned by one of our licensed dental hygienists. They are professionals dedicated to preserving and maintaining you maximum oral health.

Dental Evaluation and Treatment Plan
After your preventative appointment you will receive an outline of your dental evaluation and treatment plan, which with your cooperation, will restore your mouth to a completely healthy and functional state.

An estimate of the approximate cost of treatment will also be made at this time. You are encouraged to ask any questions concerning your dental care. Successful treatment can only be achieved when there is a mutual understanding between patient and provider.

Financial Arrangements
Payment is expected and due at the time services are rendered. This allows us to control billing and accounting costs thus keeping down the cost of your overall dental care. Other financial arrangements may be made with our office manager before treatment begins if extensive treatment is necessary or treatment exceeds you budget.

Our Office Payment Policy is as follows:

  1. 5% discount for treatment plan payment in full at first visit after consultation.
  2. Full payment is due at time of appointment for that days service rendered.
  3. Partial payment each visit with balance paid in full within 3 months of completion of treatment plan. No service charge.
  4. Partial payments each visit with 50% of treatment plan amount on a 12-month contract with an 18% service charge.
  5. All balances past due over 90 days subject to an 18% service charge.
  6. Your dental insurance will be accepted as partial payment but the differences should be paid at the time of each appointment. Almost all insurance plans cover only approximately 50% of total after all things considered.

You will be asked to choose your method of payment and sign an agreement of these terms.

Time and Scheduling
We will perform the maximum amount of treatment in as few well planned appointments as practical. Every effort will be made to schedule these appointments to accommodate your personal schedule. However, excessive abuse of appointment time will result in termination of treatment plan contract. Your promptness for appointments allows our office to operate on an efficient schedule and assures our other patients of the same efficient treatment.

Recall
We share the desire of all our patients to minimize the need for extensive dental treatment. This can only be done by regular examination that prevents little problems from becoming big or expensive ones.

After completing your treatment you will be placed on our Preventive Recall Program that requests you to return at a specified time for re-examination. This visit allows us to inspect your recent dental treatment, review your home care program and clean and polish your teeth. Preventive dentistry is the best dentistry. Don't miss your check-up! If you think you may be overdue for this appointment please notify us.
